Fantasy setting, but one where evil has pretty much already won. Magic is heavily regulated, dwarves and elves are effectively gone (and speaking to one is punishable by death anyway) and the whole thing is just about doing what little good you can to make life in the world at least a bit more bearable.
A very fun setting with a lot of possibilities, if a little depressing.
Another setting ripe for MMOsploitation would be Legacy of Kain. My God, it has everything: Time travel, vampires, worlds out of balance, humans struggling for survival, an ancient evil race trapped in a parallel dimension attempting to reclaim what was once theirs, a vengeful ancient God pulling everyone's strings, a sentient blade which devours souls, self-fulfilling prophecies, ancient magicks... it's a setting which sits around screaming, "MAKE ME INCREDIBLE! MAKE ME INCREDIBLE!!", and yet all we get is some shitty MOBA game... a game which I only support out of hope that a competent developer will see interest in the series and DO SOMETHING WITH IT.
For fuck's sake, I've never seen such an epic series & series concept treated so poorly. Then again the series requires thought, which might hurt the poor little brains of the COD tweens who make up the modern gaming community.
Plus the vampires don't sparkle.

I'm sorry, but it would be the perfect IP for a MMORPG. It would like combining Sci-Fi and Fantasy into one game, e.g., swords, shields, guns, missiles, ships, mounts, etc.

I'd suggest M.A.R. Barker's Empire of the Petal Throne ( Tekumel ) but I doubt the vast majority of people who play MMOs these days have ever heard of it. Massive setting, untold factions and subfactions, epic loot ( The Excellent Ruby Eye, anyone ? ) and Mihalli. "Nuff said.

Based on a AD&D campaign, it has TONS of source material

Factions. Side with the Golden General, or Lady Kitiara.
Multiple races. Including the "Holy Trinity," also has Kender (halflings), Gnomes, Minotaur(s?), and... Draconians. Also, three subsets of both elves and dwarves, as several human bases of origin. Plenty of conflicts to be had.
Classes. You have your traditional classes, but some with twists, like being a wild mage (a mage that hasn't undergone the Trials, as is hunted), or a Knight of Solamnia.
It was tried, but the company went under or something. It dropped of the face of the earth it seems. I always though that Stargate would be an awesome IP to become an MMO. The style and culture of worlds are only limited by the Devs imaginations. Traveling to said worlds is an instanced world, with a loading screen of the worm hole, and that horrible noise that hurts my ears (really the only thing I ever didn't like about Stargate lol). There are alien races, human races, non-humanoid races. Disasters abundant for you to strive to save. You could have one world in medievil times, and the next world more advanced than the Asgard...which brings into Norse Mythos (my personal favorite hehe). You could be a farmer on a planet, a warrior for some planet, part of the SG program, or any other numerous options

Each planet would have several Houses (guilds) that would power struggle over control of their respective planets.
Players can eventually travel to Arrakis once gaining enough funds to get a ship.
Fast travel = Paying the Spacing Guild to Space Fold
Arrakis would be a PvPvPvE zone where players would mine for resources and rise through additional ranks with the Fremen
