Shouldn't mmo's start off very ez and as you level get progressivly harder. Completly remove general level's from the game and replace that with skill levels.
So basically peaple that arn't that skilled can do like beginning to mid game stuff, but only the most skilled players will ever be able to do the top end game stuff.
Also have gear given that's relevant to the skill of that content, meaning the best gear will come through the hardest content in the game, while the eaziest stuff will just give really low generic stats.
The mmo i play now isn't like this, which is ffxiv arr. In that game i basically got a level 50 and started doing low level roulett's everyday and ended up with a full set of i100 gear. The thing is, that roulette is for dungeons lvl 15-49, and i100 is more than enough for a good chunk of lvl 50 content. Its sad i can get gear like that withought even doing anything remotely difficult. Aperently hunts in that game i think you can get i120 gear, and there is really not much challange in that at all.
I finally did the first 2 end game story dungeons in that game, and it was so ez with my ilvl, i was well over 40 ilvl's compared to the dungeons lvl at that point = no challange.

Actually game design is anti real in it's present form.
It should be a LOT harder at the start,after all you are a noob,few skills and abilities.I feel it should be really tough to get to level 10 or 20 then once you get a lot more abilities it is only tough if you are not skilled in choice of those spells and abilities.
I don't think xp should be done the way it is either,i would like to see xp even right across the board.
As for gear,i actually prefer ALL of FFXI's design,just needs some tweaks.Gear should not be auto replaced every few levels,there should be choice to upgrade that same gear and the combat should force you to change that gear depending on what you fight.Lite gear for avoidance on heavy accuracy mobs,or heavy armor to fight piercing type mobs etc etc and elemental properties added to the equation as well make for a very diverse game,instead of the so many shallow game designs i see.
It is one the main reasons i don't get into Asian rpg's,they tend to be very focused on cutesy characters and shallow combat.The best i have seen was actually in SRO,freeze type attacks,Lightning type debilitating type attacks and Fire attacks for Bleed and DOT's.Aside from that i never see any thought or diversity in combat,usually just spam combat.

main story isnt endgame , far from it...is balanced around 2.0 gear (max at the time ilvl 90 so yeah is easy)
after gearing u can do primals in hard (ilvl 80), then extreme mode (ivl 100) , u will find all them challenging even when geared.CT raids , labyrinth ilvl 55 , syrcus ilvl 80 and WoD ilvl100 (are kinda the LFR of wow) for fast gearing too (no lockouts of gear now before u only could loot once),do it for the story and for the weekly quest and some 120 gear :P
and the the true endgame Coil of bahamuth , first coil is ilvl 90 gear , second is ilvl 110 , and final is the actual challenging content 130 gear , all of them have ECHO now (a buff) but still challenging (final coil at least)
u just leveled on the last pach before an expansion and u are probably doing contenting with ppl in max gear , everything will look easy when doing content for your ilvl , since u cant probably even enter harder content since u are undergeared.
btw noo one does hunts anymore , ppl did it for months but now if u do it is for fun or cosmetic gear.
next time do some research ~~
