I always just kind of create word that sounds like a good name. By now I tend to fall back on the same few names over and over but originally I would kind of look at the race of my character, the region they are from, and create a name that sounds like it fit's a member of that race from that region. Sometimes these names have inspirations or are rooted in real words. For instance:

Andius - My name is Andy and I was playing Dungeon Siege which has characters descended from a legion of a Roman like empire. Therefore, Andius. (This was in 7th grade)

Kaemik - No real inspiration. It sounded cool and barbaric and has commonly been a name I use for red haired characters of barbaric decent... or dwarves.

Eldurian - In Ogame I would play by the name Andius and my original planet would be named Andia. However when I eventually created a larger more powerful homeworld than my original planet, it would become Andia, and my original planet would be Eldaria (Rooted in the word elder, because it's an older planet.) I eventually adapted this into the name Eldurian, though it's roots are kind of funny in that usually characters named Eldurian are roguish and immature.

Depends on the game. Often i will use Norse, Roman, Celtic,, or other ancient dialects.
