Instancing/Scaling MMO?

Hi.  I'm looking for a new(er) MMO that either relies significantly on instancing, or scaled content to the size of your group.  I'd like to play with some friends, but most MMOs fall apart past duoing.  The content becomes too easy and there is no incentive to do harder content, or its simply not possible because the quests are level capped.
Suggestions?

    I fear that the good games with challenging instanced content all are old. Guildwars for instance is exellent to play with a few friends but the look of it is rather dated. GW don't have scaling as such, instead you bring in npcs (some of which you control gear and skill on, others preset) to fill the group.

    It at least used to be pretty challenging, I am not sure today though, the nerf bat have hit many older games.

    Newer games tend to have their instances set for a specific group size (usually 5 people) and while you often can complete them with 4 if you play well things is rather rigid. Also, those games generally don't start up the instanced content much until endgame.

    The best new game with scaling is GW2 but it's scaling events tend to get a bit easy or demand a lot of people around. It also have some scaling on the instanced story. If you want something new it is still probably your best choice and the basegame is free so you could at least try it and see if it works for you unless you already tried it.

    There is some kind of CoH project and I think they have a server running (and it isn't illegal) but I don't have much details myself, just seen someone mentioned it on the forum.
