This is a thought I've been mulling around for a while, and I thought I'd post it before I forget it.
The basic components would be:

Setting: A low medieval fantasy world loosely based on post-Roman England and Wales
The game is essentially a hardcore sandbox environment with only one NPC-run starting city that players spread out from to found their own cities, castles, monasteries, etc which are progressively built up to form player-run kingdoms, each headed by a (player controlled) royal family. In addition to wars with other kingdoms, you must worry about NPC raids, either by Vikings or Picts depending on your location, and the occasional dragon that takes literally hundreds of men to take down.
The overall object of the game is to control 51% of the land in the game along with the city of Camelot (starts as a ruined wasteland infested with bandits, but can be captured and rebuilt), at which point your family is said to control the throne of England. Once someone controls the throne, the second phase of the game begins, which is defence against successive waves of well-equipped NPC Norman invaders from the south who will not cease until all of the map is brought under their control. Once this happens, the game becomes about revolution and guerilla warfare against the Normans until enough losses are inflicted that they are forced to retreat, at which point the game once again becomes about kingdom building. This cycle repeats itself indefinitely, and prevents the game from getting stale while not completely erasing the world periodically as the upcoming Crowfall wants to do.
