The Matrix Online - I loved the feel of the game minus the actual gameplay mechanics. For me the game developers nailed the atmosphere, music and tie-in to the movies. I would love LOVE to jack-in again with a TMO re-make!
Everquest - My absolute favorite game of all time. Remaster it with some minor tweaks (spread out the population from bottleneck on a few dungeons, ZEMs and loot tables) and I'd play another 20 years!
Honestly: Dungeons and Dragons Online. What an IP to have and to screw up so badly. If they were to give that license to a competent developer...can you IMAGINE the potential.
SWG - Before the NGE but with everything up to that point with the Knight trials completed and functioning and Perma-death for Jedi and Bounty Hunters.
Vanguard - Finished, polished and delivered.
If you want a new idea, go read an old book.

(Yeah, Wildstar is recent but those are the only three MMOs I've played for any length of time.)
One thing that made CoH/V so great was the character generation options. What made that possible was the (relative) low graphics. It's easier to make new items when you don't have to make them as detailed as in today's MMOs. It can be done, it just takes more resources.
-SWG: I can’t believe no one tried to make an Open Word Sandbox PVE MMO since SWG.
-EQ: I miss Norrath, best MMORPG lore/world in my opinion.
-WOW: As much as I like the cartoonish graphic, I wish they made a more realistic, gritty version of it. Possibly with Vanilla gameplay and some sandbox element thrown at it.

SWG over 30 classes u could be 2 and 1/2 at a time, JTL space ship fighting so much freedom we lost with its passing.
