Keeps (Castles), and towers for that matter, can either be "Claimed" by a guild or unclaimed and only guarded by standard troops of the realm.  You aren't guarunteed that the Keeps will have any players in them but in general, since the maps will show the castle under siege, people will come flocking to defend.

Keeps and towers exist in certain zones.  There are the large zones that everyone can go into (although it is mostly level 50's) where each realm has its own frontier.  The three realm then fight over those keeps and towers.  You can knock holes in the walls of keeps, or batter down the doors.  Towers can be destroyed to rubble.  If a keep or tower is claimed by a guild, you can upgrade them.  This will add to the number of NPC guards, make the structure larger, and increase the strength of the walls/doors. 

There are level specific zones.  These are about every 4-5 levels so level 1-4, 5-9, 10-14, 15-19, 20-24, etc..  The ones prior to level 20 are usually empty.  Each one of these zones, known as battlegrounds (or BG) will have different terrain, keep, and towers.  These zones exist for people to RvR in that are not level 50.  A level 50 versus a level 35, and the 35 is dead.  But a level 24 versus a 22 and you are more equal.
