Well, in terms of general style of gameplay, they're pretty similar if not identical. They both offer a decent storyline, good graphics and environment, instanced explorable world, a great skill system, and good PvE and PvP.
My advice is that if you want to start with a good solo/PvE experience, buy the original Guild Wars first. Just my personal opinion, but I find the PvE campaign in Factions a bit less deep; the cutscenes aren't as good, the plot is a bit hackneyed, and there seems to be an emphasis on levelling your character really quickly to get him to the high levels.
On the other hand, you get two extra professions with Factions and if you're intending to jump straight into PvP stuff, it's a fine place to start.

I did the stupid thing and decided to try out D&D:O and am one week into the trial and know I'd have been better off getting Factions. Am considering forking out for it, but am in two minds. I was bored of GW in 3-4 weeks..

So, back on topic sort of, if you get Factions, does it add anything to the existing GW content? Via skills etc?

Buy the original first would be my recommendation too. It's a very fleshed out world and story, Factions is good but very fast advancement makes it less immersive. (Though still excellent to play, esp. for those who've been through the Original already)
As for the new skills someone else asked about, there appears to be about 30 new skills for EVERY "original" class (with 5 of them being "repeat" skills allowing double-up of a similar skill on your skill bar and about 10 being new Elite skills). It's a very substantial gameplay add-on and the PvE storyline missions are designed to get you learning the "full-on" game very quickly.
All in all it's certainly worth buying Factions, but the Original should definitely come first for newcommers.
