ive been missing mmos, would like to find a good one that:

is medieval or sci-fi (but not a spaceship game), preferably 1st-person.. maybe 3rd-person... but no isometric/bird's eye or side scroller views (not immersive for me)
focuses more on user skill than character level/character skills (in other words i would rather manually aim at a target with my own skill than rely on dice rolls and my character's level)
does not mainly focus on/revolve around grinding (if you can "cap" and then get bored with the game... i don't really like that system too much)
has a big world which is not instanced.. to allow lots of player interaction (it's fun to run into other players accidently while exploring)
has fun PvE and balanced PvP

i used to love Neocron and Endless Ages, but i quit them both because of the administrations who took bad paths and ignored their communities. PlanetSide and GunZ are fun, but only being able to fight other players and do nothing else gets boring to me after a while
Twilight War seemed to be what ive been looking for, but i guess they stopped making that or something?
(the games i mentioned are mostly FPS... i'm not only looking for an FPS, but in most cases it seems that mmos that are not FPS', are grind-focused, level-based, click-target-attack based games which are just not fun to me.. but there may be a game that isn't FPS that doesnt have those bad qualities?)

I mean you've got Roma-Victor with (almost) twitch combat, in which tactics may play a significant role in larger PvP encounters, but individual effectiveness is pretty much all based on character combat skills.
But at least RV meets your first, fourth and possibly fifth requirements....
Apart from that, maybe Darkfall? I'm not sure.
Post NGE Starwars Galaxies now has twitch combat doesn't it? But it's also skill based + grind just like RV.
