Sadly your not going to find that. But if you think about it the 15$ a month is actually a good deal cause the company that owns the game usually adds content and the money goes to customer service. You could play Guild Wars but I don't consider it an MMO but a CORPG.
Guild wars is the only game that fits your description, but it hardly counts as a mmorpg, even its game developers call it something else.
As far as comepletely free games go, I would suggest Anarchy Online. The classic version of the game is free for an indefinate period of time, its home to the best in game customer service team around, and features massive amounts of dynamic PvE content, its PvP is however fairly lacking.

You could give Minions of Mirth a try, it's free to download and you can play it for free up to level 20, then it's a one-time $30 fee. Graphics are pretty similar to EQ1, has some really nice features, multiclassing, playing as monsters, flight, crafting, player-made content, etc.

But there is a catch - They make money by doing expansion - each expansion you have your character have acces to better skills (it is true) , now seeing Guild Wars is heavily PVP oriented game , you are going to find yourself outpowered and outmached , and you will want to get the expansion ...

Guild wars and many FPS' don't require a monthly fee, but guild wars does release new chapters, and these new chapters are needed if you want to maximize your abilities in game... new spells, skills, etc. Some guilds even require that you have the new expansions.
Its an mmorpg with an emphasis on pvp. Hence the low level cap and pvp ranking system. It actually has the best pve content Ive ever played and probly will ever play for a while BUT there isnt a lot of it

Right now Guild Wars is the way to go... beleive me, its worth your money...
If you try it and if you think its lacking you can always wait for...
Hellgate: london, its still in development but it will traverse in the same footsteps as guildwars... dont know about expansions, but one thing im sure is that this game doesnt have monethly fees.. and the creators are the creator of the original diablo...
so you might wanna look that up later on, cuz right now this is the only game im looking forward to...
