Major reason? No grouping, guilds, channels, or subcommunity support. Of any kind. And, as many others have pointed out before me, RS needs those badly. Really badly. If they'd implemented that stuff at the beginning, there would still be a boatload of annoying people. But everyone else would turn off public chat and use their clan channel.
Secondly, because all the elements of the skill system are totally independent, there actually isn't any reason to have grouping in a normal sense. Everyone can be a whatever-level Melee fighter, Ranger, or spell caster. There aren't any support spells or group buffs at all. In a general way, that applies to the craft skills too. There isn't really any interdependency except for lowbies needing crafted armor or for people who would rather buy fish instead of sitting their collecting them.
Thirdly, the skill payoffs are roughly linear. The experience needed to raise a skill seems to be roughly exponential. Someone said the grind wasn't bad. As if having to do one hundred thousand of anything could be good. ~13,034,431 to get level 99 in something. 120xp from the highest EXP fish in the game (not that you'd even be able to catch them until level 62). About 108,621 Monkfish to catch. For the first batch of levels, you change what armor and weapons you can use or what items you can level up on often enough that the character progression aspect (ding fun) breaks up the monotony. Like any function that is Little-O of another, the grind eventually dominates its lesser brother. Once you stop for a second and calculate how many hodjillions of whatevers you've got to do left, things go kind of downhill.
I hate runescape because you can have names like (and people do have names like), "Sup3rAZNBR", what's the point how can you be both "AZN" and "BR", I just don't understand...
Here's the thing, Runescape is the Fischer Price of MMORPG's. You try it when you're young and you think "Hey! awesome! I can play online with my friends? And have cool armor and stuff? Awesome! I've never done that before!". People who aren't new to MMORPG's just see it as a crappily made plastic toy.
And eventually, as you grow up, and try other toys, you realize it, too. And then one day, you toss the plastic hunk of crap into the garbage and pick up a bigger and better toy.

RuneScape started nothing. The only thing RuneScape started is god awful "l33t dewd" terms like "n00b".
