The games I'd have to say felt the 'best' (most complete, fewest bugs/issues) would be Dark Age of Camelot, City of Heroes, and Lord of the Rings Online. Those were by far the games that I felt were well and truly ready for release.
The worst? Anarchy Online and Vanguard...oh the pain... and I'm debating about adding Tabula Rasa to this list.
My favourite beta would have to be Planetside. Also bt'ing one of the best atm imo.
WoW beta was not bad but the lag was pretty awful sometimes and when we organised raids, more than 10 people in any place resulted in 5 mintue waits for spells and skills to happen. Also the queing to get into server was terrible.
Worst beta was probably early face of mankind, pretty much everyone was invisible and nothing worked. That's not beta, that's development.

Fiesta Online is good but it has something that could be better. Like making the monster drop some copper so u get money to buy ur equipment. Overall its a great game.
The only open beta test that I kinda like was World of Warcraft. It's not your original generic mmorpg, however after it released, and I have level up a few times, I get bored with the games after I get killed over and over again from trying to complete this difficult quest. Also I got bored of it because I am not leveling up faster, and I am not getting enough money to afford new equipment from the vendors, and the loot hardly even afford much.
Today's games that are already released for beta testing lack better features, and just becoming another old, generic mmorpg that only just waste precious time grinding, doing boring quest that does not have a storyline to it or a cutscene, and the combat system are boring. The only game that caught my eyes and waiting to test out are the new ones which will be under closed beta test are Darkfall Online, Age of Canon, and Warhammer Online. These games are the ones that I want to beta test, Darkfall Online is the game of freedom of your choice that you wanted, Age of Canon has a nice combat system and based on a old-popular movie/book/comic, and Warhammer Online because it's quite famous to the level of Dungeon's and Dragon's.
I still need to find a game that can hold me till I can get accepted into the beta test of these games, right now I am bored of playing old games such as my Civ 4, and tired of re-installing my boring games such as Oblivion, Black and White 2, X3: Reunion, and my other games (I also quit playing them due to too many technical problems with them such as lag and crashes).

Although Earth & Beyond didn't have the complex system that Eve Online runs off of, E&B had a great combat system, easy controls, and was easy enough for just about any noobie to pick it up and space out! ... Pun intended.

It's not so famous like WOW or Fantasy, though if say the best open beta, it's really successful. Great progresses have made.
Matrix Online was also an awesome game until they fucked everything up after launch. =(
