I recieved Amanomurakumo in FFXI which is a Relic Great Katana. Only one or two people have a relic weapon per server in FFXI which allows an exclusive ability to anyone who wields a relic weapon, took me 3 years to get. Man I wish I had that kind of time now to play MMOs....
I also recieved Maat's Cap which is something you get for beating Maat with each level 75 Job in FFXI. It isn't as rare as the weapon but still it's pretty rare to see.
Nice thread. It brought me back to some old games I haven't played/thought about in a while. In SWG I a bunch of rare items, though nothing I would classify as jaw dropping in rarity. Among my combat related rarities; I had a DE-10 Pistol, a full suit of RIS armor, a Trando Hunting Rifle, and a KS Firespray. I had loads of rugs, paintings, and holos too. A full set of all the JTL paintings was probably the rarest of decorative loot. In WoW the rarest thing I ever had was the legendary hammer Sulfuras, Hand of Ragnaros. I was lucky enough to have the highest DKP in my guild the night the Eye dropped off Rag. It took me another six months of farming to gather the rest of the mats needed to craft the damn the thing. Not to mention the scores of angry guild warriors I had to deal with who cried that the hammer was wasted on a paladin. Then sadly all my months of effort were made useless when BC hit and my legendary hammer was out classed by 66-68 lvl greens. That more than anything killed WoW for me. I hung around for the rest of the BC, though I never really got back into hardcore raiding. I finally left with Lich King.

The Flower Black Silk Sash in EQ1 was probably the rarest drop I had obtained in an MMO. I wasn't camping lower guk for that item in particular but more exp grinding with the group when it dropped. i had probably spent dozens of hours in that one room and was ecstatic when I won the roll. After the group was finished the SK wanted to purchase the FBSS for around 8k platinum and I was a very happy camper after that.

Holgresh Elder Beads, awesomely rare, and an absolute treasure for a monk.
Hellgate London... I got 2 Unique fusion rifles that are the most ubber guns I've ever had the pleasure of playing with. Lee's Light lance (extra critical and range), and Dunbar's Disrupter ("on-hit % multiple elemental and nova attacks). If I had a choice, I'd go Dunbar's over the Lee's.
Back in the day when I had 12sky and played it to level 25, I seen only some few people with plus 11 gear that had this rare and powerful buff from a skill book called Garudo song, a skill book for boosting your luck very high. And i was also told people flock to them and paid them to plus there gear because of that buff and they had great luck in making gold from other players even if they failed with all that luck, players did not even get mad they just keep coming back I was told. So I was like I want this skill book how can I get it I ask and they told me you can buy it off someone for 180mil gold or you can get it as a drop from mobs and that can take you a very very long time they told me. So at level 85 I got that lovely book in some cave I was leveling up in and got so excited I nearly fell out of my chair. So I shouted in game "a mistake" rare Garudo song book just dropped for me and received a total of 45 offering whispers, the highest offer was 230mil gold. I did not take the offer
For me it was Captain Nalot's Eyepatch of Plunder in the days of Velious. My character had the most ultra rare gear on the server but I was also an ultra hard core player then. That was one piece of gear that I EARNED through persistance and patience. This isn't an 4 hour camp. The Griffon you needed spawned once a week randomly. It took 3 months of trying. You can't just earn gear like this anyomore in today's mmorpgs. I also had two fo the best weapons in the game for my class, Tolan's Longsword of the Glade and Swiftwind, a full set of Vellious armor, plus a few goodies from King Tormaxx.

In EQ1, Pre-Velious I had FBSS, J-Boots and was one of the first to finish the Soulfire quest. Not earthshattering, but I felt accomplished.
An epic staff that dropped during the death knight quest or Greatsword of the Jin I think its called dropped the day I hit 80 on my death knight.
