World of Warcraft .----> Both in design as in support and follow up. Just take your personal flying mount and fly over the seamless lands of Northrend ... EVERY flag you see is animated in the wind, every bird, rabit, rat and living creature is animated and breathing life. The world setting alone costed probably more than some full blown mmo's in its total development costs. Full passable 3D background loading and design costs roughly 3 times more than the older traditionel settings of "zoning". ---- Follow up: 1 month (!) after the intro of the cross server dungeons, Blizzard already adapted mulitple dungeons in accordance with the analysis of their server data concerning player activity. This game is the MOST analysed data mine ever for balance and gameplay issues. I doubt we will ever see such an over-analysed game in the future.

I want to mention that Blizzard made excellent use of test servers for players. I hope other MMORPGs look at WoW's methods of making sure updates are tailored to their players because it shouldn't be about "WoW has this, we don't, so let's add this".
LotRO has been brought up a couple of times. I thought LotRO was one of the most polished MMORPGs during its launch and first year, but does everyone think it's the same case with recent updates?
Edit: I don't even play the damn thing, but holy hell is it just a pretty game and well detailed. Follows lore and is just what MMo fans who wanted to play in Tolkens universe could have dreamed for. I do say bravo on this one sparkle in a bleak crapshoot that is a WoW dominated genre.

Middle Earth in Lotro looks like Canada and Middle Earth's forests never were described as canadian woods.
The escale of the World doen't express the feeling of Middle Earth, it is so reduced that places that should be far away from each other are just a couple of seconds ahead.
Classes are generic and do not express any soldier, hero, character on Tolkien's work.
The artstyle used on characters and gear is poor and it is hard to understand how as there is a lot of good and expressive art related to Tolkien's books.
They turned some lore aspects that should be darker and more significant to Middle Earth into simple and ordinary quests, not even heroic long storylines.
This is why I prefer original IPs, the world of Guild Wars, Warcraft and Everquest are universes that fit well into a game and still have the "feeling" of a world. When companies get the "world" of a book/movie/poem and convert it into a playable world you usually get the feeling of a simplify universe. Turbine did this with Lord of the Rings and Dungeon & Dragons just like Mythic did it with Warhammer universe.
So saying a 6 year old game is more polished than say a 2 year old game = win? If a developer doesn't know its engine and game mechanics after 6 years of release status, that would be a fail.

I'd say EvE and Lord of the Rings Online. I dont play either anymore,but might give Lord of the Rings Online another spin for fun.
In terms of being polished, EQ, EQ2, and WoW are the most polished MMOs to date.
Polished means exactly how it sounds. You don't polish a car until it's absolutely flawless and clean on the exterior.
Some think polish is isolated to exterior, but if your definition of polish includes the interior and rich content, then:
