The thing about MMORPGs is that you have to "Improvise" (I.E "Roleplay") If you want to challenge yourself in any way. There are lots of creative ways to make the game challenging by doing this. For instance, in WoW there was a well known player who made it to level 70 using no armor or weapons.
Just because the options are there, doesn't mean they are necessary.
Every try a "Melee" Hunter? Or what about a spell-power stacking Rogue back in the day when Weapon procs benefited from spell power?

MO, Darkfall, and EVE seem to be the closest games to what the OP is looking for. WoW is the opposite of survival. I haven't played it but being another EQ-clone I can assume that I cold create a character and log in, create a simple macro to hit ALT+1+2+1+2+1+2, etc. and come back later to find myself a few levels higher . ... sure games like Guildwars may have a title that can only be earned from never dying, not even once, in PVE but I do not think that is what he meant by survival.

I've recently got in touch with Dead Frontiers. They're working on the Unity3D engine to bring a top down browser game. Mostly a zombie survival with consensual PvP.
