Yeah, used to do the same thing. After a while of playing WoW, I found a few race/sex combinations that had really nice spell animations. Female Undead/Human for the most part.
Anyway, I generally play humans simply due to the fact that in most games, they have some advantageous racial stats, or at the very least, aren't at a disadvantage in some areas like other races are. And, being a male, I only play males. Playing a female toon to me is like playing with Barbie dolls as boy...it's just not natural. And any guy that says they just do it so they have something to look at is a sad individual. If you want something good to look at, log off of the game and log onto a porn site.

The names either comes to me or I browse out a fitting name, usually a old norse or gaelic name unless it is against the setting or the character race.
One of the choices you didn't really have(like what was sort of said above) is "Whatever character is more popular with Groups". I nearly always go with a DPS character, but of course Tanks and Healers always have a spot in groups while DPS characters are generally much too common. Since Healers are always in demand I could start the game with one, but I usually prefer my main to be a DPS and I like to learn the game before I create an alt for this reason.
Talking about this stuff really has me missing Ultima Online. My main started as a Blacksmith/Miner, and over the years became a swordsman, and finally ended up a Mage/Tamer. I had to sort of start from scratch each time but it never really felt like I did because I could juggle so many skills at once. It's truly bizarre that the level system became more popular than the independent skill system.

The first char I create usually try to make it look like me in a way (tough I haven't found a game that lets me create a fat character, though), alts are usually created going for a wacky look, and usually trying to pick up options that are most likely to be the least used (like a balding hairstyle and the like).

For a assassin or similar, I like to choose a mysterious slim race, a warlock type class I usually choose a dwarf...lol I don't know why for that one, but yeah. I then try to make a good looking person for most characters.

The Gender i chose is an aesthetic decision and also classbased, some classes have a male, some a female preference but sometimes its fun to break the preference and make the oposite. I would never play a hobbit and crap like hobbits, i want all hobbits dead - they be no truthfull midget race! The look is very urgent, cant play a game when i cant stand the look of my avatar. For example i would only play male Dwarfs in Lotro and only females in AoC bcs of the look, while i absolutely like Trolls in Daoc. I voted Race and Lore bcs it fits the best - Aesthetics can be added to that.

Well since you didn't put Skill Damage abilities.. I'll go with Race and lore, why? because every race has their special ability and i always look onto the best class with the best ability that i can use in my future gaming.
