Hardcore MMO Player: "Harsh death penalty or permadeath!"
Everyone Else: "No death penalty!"
You can't please everyone. You can only make a game and hope it pleases enough people to be profitable.
Personally I don't see why they can't make a hard mode that goes for more than individual missions. They could allow the player to set it at a per character or per account option. If you want Hardcore perma death...set it for that, same for death penalty or no death penalty. Leave it up to the players. I am sure not everyone would be happy as that is impossible, people love to bitch, but it would be an answer.

For as much as I think we need ground breaking change in MMOs, I am really glad they are not created like we have demonstrated in this thread. Such a MMO would really deserve the name Anarchy Online...damn it the name generator says that name is already taken.
The perfect MMORPG for me right now is the "In theory" Revival MMRPG.
But from what I think, it's awesome if we have a open world game, without a map or a compass (unless you buy one), but, even if you get a map, it doesn't magically show your position in there.
This simple thing makes you think... where do I need to go now? Certainly, the answer lies within the NPCs or players near you, in the village, city or wherever you are... But... these NPCs doesn't have a exclamation mock jumping in their heads, begging for attention.
Lets take a look at Neverwinter Nights for example (NOT the MMO, but NwN1 and NwN2), quests descriptions are amazing! They "kinda" tell you what to do... but not WHERE to go.
I think about it as some sort of survivor thou... but not hardcore, as the game would be Open World, there would be OW-PvP without restrictions, taking real life as an example, anyone with a weapon can attack/kill/stab you, but the common sense or security don't let them do it at will.
I could still be writing everything I'm thinking right now, but as I'm lazy, i won't do it. XD (surely a "For you, how could be the perfect MMO?" would make me write a lot of pages here)
User generated content that is rated by the community. If popular and balanced enough, the top few get added to the game as permanent content and the creator(s) are rewarded.

Main thing for me is action-oriented combat. I simply cannot play games like ffxiv or WoW anymore after playing games like Smite and TERA.
