yeah i'll believe that when i see it from ZOS not some 3rd party market research company... not to mention i for one paid for a 3 month sub which is still active.. although i havent logged into the game in like 2 months.

And if that is what you meant, non-phishy surveys don't need to and almost never identify the person surveyed. Apart from the fact that in almost all cases it isn't necessary, people are more likely to provide honest feedback if it's anonymous. In this case it probably doesn't matter either way but I could name several less popular MMOs with staff who behave vindictively towards anyone who criticises them.
Even in cases where identity is important - eg. longitudinal studies where you survey the same group people at different points in time and you're interested in individual changes - participants usually get anonymous IDs.
The headers looked legit and seeing as how some managed to do it, it seems like it was. Maybe there was a problem with the survey and they pulled it down early.
