Look, I'm a simple man with simple wants. Just leave out obviously stupid skins. No Santa and Easter Bunny suits. No rainbow unicorn bows. No disco ball maces. It should be obvious when an item just blatantly doesn't belong in your setting, much less as an armor or weapon.
And also, yes. No built in, non-consensual voice chat. Nothing takes me out of it faster than hearing some randos voice.
For me only 2 things come to mind - seeing an annoying cash shop button, or when I realize a game has auto-pathing, where you click a key, and it walks you to where you have to go.
Every holiday event or other IRL linked events, inappropriate skins or mounts, UI built in cash shop button and gamble equipment enhancement system like the ones used in Black Desert, Blade & Soul etc.

Missing wandering NPC. Damm for the Love of god, noone stays around the same place like a statues for Months. Let them move a few steps. Even better if they are at total different Positions as the day Progress.
That Counts for NPC's in Cities and out at some encampment or small settlement. Heck even Monsters, noone stays in one position forever.
Also.. i really dont like he fact that you get the same "Trivial" loot regardless how the critter said goodbye to the world.
It would be great if you get "Bone Chips" from critters that you Smash, "half a Bone" if cut and surely no Pelts when you use a Slash weapon on them.
Also too many Pop up Messages.. God i dont Need 2 or 3 GIANT sized Pop ups whenever something MIGHT come up that warrants a help window ..

Experience given for doing quests,your character should not be more experienced at their role just because they did some fetch me quest.Your character role should be tied into your skills and those should be used to get better at them.Pretty simple concept really,if your mom sent you to the store to get a loaf of bread would you then become a better Warrior?
Enemies should only drop loot they are using and especially not drop something stupid like mounts and pets unless of course they happen to be using that at the given time.

I am also not a fan of out of place mounts and armor. Not a fan of a tank wearing a bikini and sunglasses to a dragon fight or a flying mount that has no business flying. Everything added including cashshop stuff should be fitting to the game.
It's like trying to isolate which, from among a thousand fireflies on a summer night, is the brightest. My immersion is under constant attack both from within the game and without. From NPCs with glowing punctuation floating over their heads, to my wife asking about some chore I promised to do. It is very hard to hang on to my suspension of disbelief. But if I absolutely must pick one, I would say daily quests - fighting the same mobs for the same reasons and rewards again and again and again.

Combat ladies and gentleman. Slash, swoosh, ping, whirl, blast blast. POW. Looks like a damn cartoon not an experience. I can see stupid effects in an arcade case.
When combat boils down to avoid the large red circles... telegraphs are fine, but make an appropriate animation, and don't overuse them.
When a quest / NPC tells me something is going to be difficult and it's a faceroll... I know most MMOs use vertical progression, but enemies within your level range should at least present some challenge / danger.

No mounts/costumes/skins that don't fit with the world. No santa hats and no steampunk cars, unless it's a santa world or a steampunk game.

Inventory is a whole animal in itself. No, I shouldn't be able to carry 100 stone blocks in my pack, but is it realistic i can swing a hammer 6 times and get a perfect stone block out of a quarry? But at the same time I don't want to play Inventory Tycoon all day either, that's hurts more than realistic burden values.
If you look at the history of mining and logging and such, you see it took a lot of expendable men and a lot of mechanical advantage to get things done. A lot of horse power, whether from horses or not, is needed. That's not realistic either. I can't be expected to team up with others just to get some bags of coal or iron. I don't have an army if immigrants looking for a new life in a new land. I have a backpack, and hopefully a mule or a cart or both.
Meaningless and/or repetitive quests. This is a biggie. It shows me, right away, whether my time is valuable to this game or not.
Meaningless and/or absurd loot. Why did this saber-toothed vampire cow drop a chipped ruby and a studded leather epaulette?
everyone above had the basics down. In general for me the insanely oversized weapons that are common in MMO's, and of course along the same vein the ridiculous armor that would never work like the giant stuff for males, the little bikini thong stuff for women. Didn't see mentioned above but environmental invisible walls drive me nuts, and inclines that aren't steep but you can't somehow go up... no those are just so bad.
I hate to see gambling system where every time some one lucky will get all the nice/rare loot. It shouldn’t be based on luck. It should be based on effort/skills maybe effort to help others.
I can tolerate a lot of nonsense in mmorpg since gaming is not reality and as long as the game is fun to play stupid things don't bother me but when it stops being fun everything bothers me and immersion breaks.

And there is immersion in terms of how realistic the world and your character in it are. By realism I mean is it what it was intended to be?

Recently I left FF14 becouse of how unimmersive I found the game.
Mounts, clothes and UI were the biggest offenders. In your face UI (do I really need giant You accepted a Quest text after accepting it?). Mounts which look out of place all the time. Everything done in the name of cuteness. When I went to check out housing I found a medieval japanese castle standing right next to a modern looking house. Everything in that game felt out of place.
The moment developers start puting things into a game just for a fan service's sake it's the end for me.
On the other hand good weather effects (mist after a rain or in the morning) are a very good features.
