Gives incredible power to an ambitious warrior, including eternal life, but there's a catch, everytime he dies he becomes more powerful when he returns, but the pain of every death he suffers is driving the man insane.

Zakalwe is definately up there. i guess what takes him down from that top notch of real eveil is that he is repentant in that his own mind rejects that evil acts that he has performed.
As to the Malazan books, i was trying tio think of someone truely evil from them, the problem is that all of the characters have really complex motivations. Even the crippled god, his origins make him a sympathetic if astoundingly tragic character. but without a doubt, the best fantasy literature out there is this series
