It depends on whether you count Island of Kesmai, Kingdom of Drakkar and Neverwinter Night (on AOL, not the new one).
Those are not massive with thousands of people, but at least they have hundreds. I usually include them at least as pre-cursors to MMORPGs.
... Oh yeah... also if you want to fix MMORPGs stop worrying about balance! Make COMPLEX characters that have bad options and bad combinations of powers. Some people WANT freedom, you stupid communist devs. I remember when Asheron's Call (The best game ever to be made) had really really bad skill choices available... you know, like a Mage that didn't have War magic until 26th level!! OHH yeah! that's right taking that horrid option might just be good for some people.
Wica. PvP killed unbalance. And we need PvP because is a replacement from content. You put PvP in a game withouth content, and bam, users have something to do. Is also needed, because some people do like PvP, and you needs lots of people to pay the artist.
PvP people all that want is e-sport, artificial shotters totally balanced where the most skilled win. Not games: sports.
PvP guys can be also carebears. Wen a PvP guy died because a small real of prenteded inbalanced, the nerf banner show to balance this again. So everything that is unique is removed.
