I've been scared by quite a lot of monsters in WoW, yet I can't recall any special one so I'll just stick to mention that. Otherwise I got freaking scared when I was running around in a big open field thinking of something else, probably what build I should run instead, when suddenly out of nowhere comes three (or four) big scary monsters with knifes that kill me instantly, without me even noticing it. That was scary.
Cant remember the mob that did it , but during my first week or so into EQ there was a mob outside the gates of East Freeport that would blind you and the whole screen would go totally black for what seemed like an eternity, while continuing to beat on you.
Maybe not jumped into the air, but I've deffinatly had my share of adrenaline rushes and panicking in Ryzom. There's just something about watching thousands of building-sized insects swarming over the horizon, mandibles clicking, roaring, killing everything in it's path, and when you finally come back to your senses they're probably close enough to rip you apart once you decided discretion might have been a better plan that day. Never have I had the pleasure of experiencing that same feeling in another game.
Probably the only game that got me wildly punching buttons while going 'OH CRAP, CRAP, CRAP, CRAP, ....!' when you got ambushed once again by some sneaky Kitin scouts at 3 am, much to the annoyance of my housemates.
The Ghosts / Bogys in FFXI scale the hell out of me - in a fun sort of way - cos every time they catch me unawares I know It's pretty likely going to end up with my death.
Suddenly they'll spawn out of nowhere in the night and in a moment you have to get away as fast as possible. You here that litle noise they make to let you know they've targetted you and you're already running for the zone. Plus they just look weird.

ORCS scares me the most!

I agree with this, especially when you're still a newb exploring new zones. :-)
