Mass Effect MMO hands down. I was in awe from ME series and if it was online with bigger universe to explore and 100 times more content and more freedom, I would not need any other MMOs.
Also Id love to see Elder Scrolls MMO world on Oblivion engine, but Ive heard it is in development
Dragon Riders of Pern by Anne McCaffrey everything is in those books to become the next MAJOR MMORPG, someone just has to pick it up, Same goes for Crystal Singer "Killashandra" what a Great MMO it would make.
Very few things would be cooler than engaging in giant mech combat with the rest of your clan for planetary dominance on teamspeak while listening to some serious music.
I know APB crashed and burned, but I would still like to see a true grand theft auto style sandbox mmo. where you could roam the city and really do anything you want.

yeah but the grind to get the roofies to put in the cookies is a pain in the ass....not to mention the quest to get the stuffed animals has too many OP enemies, err...cops...
I thought about maybe just for the fun of it a Fate/Stay Night MMO but I don't think it would ever work well with large numbers. You could have the tournament of the holy grail as a event similar to boss battles simultaniously in different areas and you just run around as a mage the rest of the time but don't know how well that would work either.
Battletech/Mechwarrior - sandbox style where you can choose to to go off and form your own clans and control systems
Halo - MMOFPS style, with base building and ground control. Call in drop NPC dropships to take you to the fight.
Prince of Nothing series - obligatory fantasy post. Really dirty, gritty world, partly based on the crusades. You'll find no paladins and elves here, only abuse at the hands of fallen angels, and a royal beat down from the Emperor. Or a gnostic mage.
