I did not vote, as none of them really strikes me as "all that and more." I am not a combat oriented player. I have fun with combat, but games should be so much more than "just combat."
Here is something that most will probably laugh at, but Wizard101 has a truly innovative combat system for an MMO. For those who do not know what Wizard101 is, think of Harry Potter meets Magic:the Gathering. You play the role of a young wizard that goes to school (for spells) and have a deck of spells that you build from the spells you learn. There are many schools of spell learning including Life, Fire, Myth, Balance, Storm, Death, and Ice. Your wizard chooses one school in which to "major in" (primary) and one school to "minor in" (secondary).
Combat in Wizard101 is turn based. Yes. Turn based. Each little wizard gets dealt a hand of 7 cards from their deck of spells to begin with and play 1 card each round. Each round, they play a "spell" from their hand. Each spell has a chance of success or failure. Strategy takes the form of "Do I go for the quick kill with my powerful yet less than stellar attack (low % success) spell" to "Can I make it through another round or do I heal now?" to "Let's debuff the monster to make him susceptible to my fire attacks."
The way combat is entered is to get within "aggro range" of a mob. A magic "Dueling Circle" appears with 8 slots, 4 for wizards and 4 for monsters. Another wizard may join this duel at any time, as can monsters. The "duel" is finished with one side or the other being vanquished.
No button mashing. No tab targeting. Strategy is involved. The pace is probably too slow for many MMO players, though. Unfortunately, combat does not make the game. Wiizard101 fails on some other aspects for my enjoyment. This system works as everyone is a wizard and there is no other form of combat to consider. I don't know how this could work for melee/ranged combat, too.
I just thought I'd throw that out there as another example of combat available. I know... "THROW IT BACK!" lol

Personally the only modern MMORPG trend that annoys me is the idea that MMOs need to be designed in a way to attract people who don't actually like MMOs. Which to me makes about as much sense as someone trying to figure out a way to get vegetarians to eat at their steakhouse. - FARGIN_WAR
