It's alot easier to create a single player game and add small scale pvp multiplayer than it is to create a full mmo with a persistant world (being a fully persistant or zoned like most themeparks), where an unpredictable number of players run around the game.
Can you imagine Dark Souls if hundreds of people where in the same gameworld as you? Killing your bosses, taking your loot, etc?

But you don't need a persistent world for a MMO. Just do what DDO or WoT does. Put all the content in instances.
