When I think of really PVP focused games these come to mind. Darkfall - Really its only point, no one will ever say they are there really enjoying the PVE. EVE - While entire game is focused on PVP, you can spend an entire gaming career doing nothing but PVE if you chose to.
Er...now I could list a lot more games like Aion, Warhammer, AOC, etc that all have PVP to varying degrees, some more than others of course, but I don't consider any of them to be hardcore PVP games, mostly because dying doesn't really hurt much in them, which takes them off the list for me (but not others)
OP did not specify hardcore PvP as a requirement and also did not want total failures and I would say Darkfall falls into that category.
Besides, the PvP combat in Darfkall is horrendously clunky and the game has a terrible UI. So I would certainly not consider that game a good PvP game.
My summation:
Eve = Hardcore PvP but the PvP is not readily available. Space is big and it is hard to track down and kill people. Most fights are unbalanced. Joining a 0.0 corp is a way to access PvP but there is no telling how often wars happen. Even so, I would say Eve is the best hardcore PvP game available.
WAR = Casual PvP which is very readily available. You can join scenarios anywhere and it gives you good exp so you can level just by doing PvP. However there really isnt much else, good, PvP besides scenarios. End game PvP is garbage, or atleast was when I played 4-5 months ago. But still the game is the best casual PvP MMORPG around.
Aion = Casual PvP which is not readily available. Kinda like WAR but without the scenarios that makes the PvP readily available for everyone, everywhere. PvE is better than WAR though and so is the gfx.
Guildwars = Not an MMORPG.
Darkfall = Hardcore PvP which is not readily available (unless you enjoy ganking noobs) but also bad game. Gfx, UI, combat mechanics, exploits is all bad in this game. So OP I would stay well clear of that game if I was you.

There is only one expansion and it's purely optional. The other 3 parts to the game are campaigns which are stand alone. If all you wanted to do is pvp then just buy the original game and get the pvp pack (gives you elite skills and such for pvp characters only). You can pvp right after installation. It isn't a mmorpg in the traditional sense but outside an FPS or RTS it is the most balanced competitive online pvp game available.
I also suggest Darkfall. That is if you enjoy meaningful pvp in a huge open word. If not then I suggest you try something with instances/battlegrounds and safe zones.
I downloaded Eve, got the free trial, but during the tutorial, thought "fuck this lol I don't want to be a space ship" :XD I'm not dis resepcting that game fo course, it's a matter of personal taste. gona check Guild Wars for free trials

Check out Vendetta Online for a space sim with great PvP. None of that, "roll the dice stuff" it's all twitch combat. The game currently has a 21 Day Free Trial:
Face of Mankind or Roma Victor is the most PvP focused MMORPG. They both are literally all teamwork to build alliances and gain power in your own faction, then duke it out in PvP. There's literally no PvE.

After having played them all I think Darkfall is the clear winner for best PVP MMO at the moment. If you enjoy PVP the most you owe it to yourself to try it.
Helloes, I am very dissapointed in AoC and am actively looking for another MMORPG to play. However, AoC set the standard pretty high with the combat system (personal opinion of course) and I could not go to a game where you point and click. Priority is PvP but I hear so much conflicting information about different games... here are some examples

ignorrant t*at. darkfall like warhammer? not even close! darkfall is centered around pvp and the dynamics that influence pvp battles and sieges. warhammer most usually uses instanced pvp zones... they are totally different, not even to speak of gameplay! those bad friends of you that played probably didn't even want to try to learn it, it has a steep learning curve, like most of the people. im no fanboi of darkfall i don't even play it anymore, but comparing it to warhammer is like comparing a frog to an asteroid field.

I resubbed to DAOC and it was dead dead dead, hence why they have been merging servers for the last couple of years... DAOC was great in its time but that time has now passed. Darkfall is the new premier PVP MMO for now. Also saying DAOC has better graphics than DF is pretty insane... something tells me you've never played Darkfall, lol.

Okay, then maybe try The Chronicles of Spellborn - PvP server.
It has PvP 'lakes'. I like the PvP there in that you don't have to PvP even in PvP areas.
A couple of nights ago I was in a PvE/PvP area and met another player that was from another house - we could have fought - but we decided to work together.

if you're looking for competitive PvP, nothing comes close to Guild Wars.
However, it's more like diablo with PvP-focus than an actual MMO.
Guild Wars 2 will be a real MMO, so that one is the winner for me.
But If you're looking for sandbox / world PvP, i guess EVE would be a better choice.

It's the same movement system as DotA except in full 3D (instead of fixed camera 3D)

If you don't care about graphics Ultima Online still has pretty decent PvP and by what you said it seems to be everything your looking for.
