Anything pre WoW. Ranting but WoW changed MMO's from worlds to explore to a game that you are lead around by the nose. Personal favorites are EQ and AO. Yes there are quests but they are more focused on just killing stuff.

For the type of visceral combat it sounds like you are after I would say AoC or DDO. Those 2 in particular aren't the typical 'sit there and swing for minutes' type, they are much faster and more engaging.
