3rd is a three way tie, I would have a hard time picking one, all three were groundbreaking innovations in MMO's IMHO, and games I spent many hours/months (or years) playing.
The game was just awesome there was so much to do trading, mining, exploring, crafting, opening a shop, your own home. I could keep going. Damn fine game.
2 - WOW
The master must be in my top three. The game is polished and the lore is in depth, with a huge player base this game has taken many of my hours.
.. A nod to Darkfall, because although it wasn't "for me", I recognize what it was attempting to accomplish, and despite some issues, I think it did it pretty successfully.
In any order, but with reasons to put them on my list
Lineage II - First MMO I ever played. I love the mechanics and don't care about the grind. Pity the game is close to dead :'(
EVE online - Best sandbox out there. I quited because time-based skill acquisition is a killer for someone with ADD 8-)
Lord of the Rings online (pre-Siege of Mirkwood) - IMO the best PvE game around where the IP is used in an excellent way. Remove the Skirmish missions and Monsterplay and the game is perfect ;-)

EQ
Original SWG
Third one, dunno. I've played a lot of ok games but i couldn't get fully attached to their world. I'd have to give it to Lotro for the Shire zone.
UO
DAoC
CoH

1. DAoC - Love, love, love my bard.
2. LotRO - Love roleplaying, love the exploring, love the atmosphere.
3. Vanilla WoW - it was just epic, new, refreshing experience. Then came the expansions.
SWG - The freedom in that game was nice
LOTRO - It's beautiful, fun and the community is great!
WoW - If nothing else, it is a huge commercial success and everyone, even non-gamers, have heard of it.
1) AC
2) Tied: UO and DAoC
Nothing else has ever come close to those three.
CoH: Superhero genre, robust character appearence options, fast travel powers.
Daoc: Norse realm for the win! I love norse mythology. My first mmorpg for nostalgia impact.
Horizons: Superior crafting, robust multiclassing, Dragon playable race.
Playing: Rift, LotRO
Waiting on: GW2, BP

1.Soul of the Ultimate nation
2. WoW
3.runescape

WoW (Vanilla & BC) - Battleground premades were extremely fun and raiding content was about challenge instead of grind.
Shadowbane - I had a blast, especially during some epic sieges.
Honorable mention to AoC - Love the world, but it's not a top 3.
Aion 2.0 seems to have a good shot at making my #3, though 2011 promises an exceptional harvest.
1. Anarchy Online, pls a sequel
